Periodic Health Assessment (PHA)

The Periodic Health Assessment (PHA) is a screening tool used by the armed forces to evaluate the individual medical readiness of their Service members. It can be conducted alone or can be combined with other individual medical readiness needs (e.g., dental exam and immunizations).
The PHA is an annual health assessment to be completed by all Navy members. It can be accessed by using your DOD ID and password or with your CAC at any time.
Through the system you will also be able to see any past PHA you have created, both certified and uncertified, as well as the certification progress of current assessments.

Quick Reference Instructions

Scheduling your virtual PHA can be done in three easy steps:

Step 1. Go to https://eha.health.mil/eha
