Periodic Health Assessment (PHA)
If you receive care at Branch Health Clinic Norfolk, follow the steps below to complete your PHA. Please read all steps before beginning the PHA assessment.
Scheduling your virtual PHA can be done in four easy steps:
- Step 1: Go to https://data.nmcphc.med.navy.mil/pha/Index.aspx (CAC Card Required)
- Step 2: Complete Part 1 of your PHA.
- Step 3: Call 1-844-863-3236, hit option 1,
- Step 4: Tell them you belong to Naval Medical Center Portsmouth and schedule your virtual PHA!
You may also utilize the option below:
Step 1: Go to https://data.nmcphc.med.navy.mil/pha/index.aspx and complete a Service Member Assessment. First time users will need to register a new account. If you have previously initiated, but not completed an assessment, continue that assessment rather than initiating a new one. Ensure that you work completely through all screens of the assessment.
NOTE: There are additional confirmation screens past the point at which you can print your assessment. If those are not completed, your PHA cannot move forward. You do NOT need to print your assessment. You will know your assessment is complete when you see a check mark in the circle above “Service Member” on your ePHA home screen.
STOP! If you are NOT a patient at Branch Health Clinic Norfolk, DO NOT complete step 2. Instead, contact your medical care provider for instructions regarding completing your PHA.
Step 2: Call the Hampton Roads appointment line to schedule your PHA at (866) 645-4584.
