Family Medicine
The Patient Centered Medical Homes (PCMH) within the Family Medicine Clinic at NMCP uses a team-based model based on the premise that the best healthcare begins with a strong primary care foundation, accompanied by quality and resource efficiency incentives. Patients in a PCMH have a personal provider, who along with his/her team, provides continuous, accessible, family-centered, comprehensive, compassionate and culturally-sensitive health care in order to achieve the best outcomes.
In May of 2010, the Naval Medical Center Portsmouth Family Medicine Clinic implemented Navy Medicine’s new model of primary care services known as "Medical Home Port". As with civilian models commonly known as Patient-Centered Medical Home, patients can expect enhanced continuity of care, increased access to care, and online appointing via www.tricareonline.com.
The hallmark of Medical Home is enhanced continuity of care with a focus on prevention. Therefore, patients will be assigned to a Primary Care Manager (PCM) supported by a team of nursing, support, and administrative staff who work together to manage the health of their enrolled population. When you have a non-emergent medical need, your PCM is the first stop you should make.
If your PCM has no availabilities that day, another provider on that team can see you. This continuity of care is more personal and better coordinated because you are known by the support staff that interact with you every time you visit your Medical Home. You are seen by the people who know you the best, rather than having your healthcare needs met at multiple different locations and sites. The risk to not using your Medical Home is that important medical details get missed during multiple and scattered visits, placing you at risk for poor outcomes related to uncoordinated care. Not to mention, there’s no place like “Home.”

Women’s Health Specialty Services
Our providers provide women’s health specialty services on a referral basis. Please see your Primary Care Manager for details.
Clinic Policy
Quality health care requires the full attention of the provider and the patient. We provide pediatric care however; children should not accompany adult patients to clinic appointments, unless another person is available to care for them.
Separations and Retirement Physical Exams
Appointments cannot be booked via Hampton Roads Appointment Center. For patients enrolled in our Medical Home Port, report to Family Medicine Clinic to pick up a physical exam checklist and form packet. Appointments will be booked by clinic staff once paperwork and labs are verified complete.
PARFQ/PRT Screenings
Member must bring PARFQ, PRIMS printout and SF600 to the appointment. These forms are downloaded from PRIMS.
