Electronic Periodic Health Assessment (ePHA) for Individual Medical Readiness (IMR)

IAW with SECNAVIST 6120.3, all active duty and active reserves service members must complete an ePHA for their Individual Medical Readiness.

You can find your IMR requirements on Bupers Online (BOL) or Marine Online (MOL)

Steps to complete ePHA

1. Sign up

Go to the ePHA website and register for an account: https://data.nmcphc.med.navy.mil/PHA/index.aspx

2. Complete your Health Risk Assessment (HRA) and Electronic PHA Questionnaires

Complete the Health Risk Assessment.

3. Notification

To notify the clinic of HRA and ePHA completion status, email your UIC/RUC, DOD ID and work/cell phone number to
usn.hampton-roads.navhospporsva.list.bhcoceana-pha@mail.mil
****Ensure email is correct and send as email receipt to receive confirmation of delivery****

4. Appointment

A clinic staff member will screen your medical history and call you within five business days to schedule your ePHA phone appointment and inform you of IMR required components. Call the ePHA office or front desk (757) 953-3948/3778 with further questions.
