Electronic Periodic Health Assessment (ePHA) for Individual Medical Readiness (IMR)

IAW with SECNAVIST 6120.3, all active duty and active reserves service members must complete an ePHA for their Individual Medical Readiness.

Steps to complete ePHA

1. Sign up

Go to the ePHA website and register for an account: https://eha.health.mil/EHA/ 

2. Complete your Health Risk Assessment (HRA) and Electronic PHA Questionnaires

Complete the Health Risk Assessment.

3. Appointment

Once you have completed Part I of your Electronic Health Assessment, please call the VIPRR Clinic at 1-844-863-3236 Option 1 or call the Hampton Roads Appointment Center: 1-866-645-4584 to complete the ePHA Part II with a healthcare provider.
