Please note: The Branch Health Clinic Norfolk does not have sick call hours or walk in appointments. All walk in patients will be assessed by a Registered Nurse and booked an appointment as appropriate for their condition.  Same day appointments are available on a case by case basis. 

Patient-Center Medical Home Port (Primary Care) PCMHP

  • The Patient-Centered Medical Home Port is a healthcare setting that facilitates partnerships between individual patients and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where patients need and want it, in a culturally and linguistically appropriate manner.
  • The hallmark of “Medical Home Port” is enhanced continuity of care with a focus on prevention. Therefore, patients will be assigned to a specific team that will include their Primary Care Manager (PCM), two to three other credentialed providers, registered nurses, licensed practical nurses and several hospital corpsman or medical assistants. If the PCM is not available, patients may be scheduled with another provider on their team to enhance continuity of care.
  • Remember that Medical Home Port is partner in patient care and is here to assist patients with all of their health care needs; however the patient remains the manager of their care.

It is the Military Health System patient portal and is designed to provide online capabilities and information for beneficiaries of the TRICARE program. Some of the services that can be accessed through TOL are the ability to make appointments, refill prescriptions, and view up to five years of personal health data via the Blue Button.
