Periodic Health Assessment (PHA)

The Periodic Health Assessment (PHA) is a screening tool used by the armed forces to evaluate the individual medical readiness of their Service members. It can be conducted alone or can be combined with other individual medical readiness needs (e.g., dental exam and immunizations). 
The PHA is an annual health assessment to be completed by all Navy members on their birth month. It can be accessed by using your DOD ID and password or with your CAC at any time.
Through the system you will also be able to see any past PHA you have created, both certified and uncertified, as well as the certification progress of current assessments.
 

Quick Reference Instructions

Scheduling your virtual PHA can be done in three easy steps:

Step 1. Go to https://eha.health.mil/eha

Step 2. Complete Part 1 of your PHA.

Step 3. Call the Fleet and Family Medicine Clinic (for those whose PCM is in our clinic, all others should see their PCMs at their respective BHCs) at 757-953-0515/2423 to schedule your virtual appointment


Step 4. Call to make an appointment.

Detailed Instructions

Step 1: Complete the online PHA survey https://eha.health.mil/eha

  • Have your DoD 10-digit ID number and your 5 digit Unit Identification Code (UIC) on hand.

  • Follow the instructions on the website to register a new account if you are a first-time user.

  • Once registered, log in and complete your ePHA.

  • Once completed, a customized education form is generated. You can save, print, or email this document for your personal use.

Step 2. Print your Individual Medical Record (IMR) Report.
You will need a CAC card reader in order to access your IMR Report.

Step 3. If you are due for a Post Deployment Health Assessment complete the following survey: (https://eha.health.mil/eha)  

The site is CAC enabled and allows users to associate their CAC with their account.

  • Members will either select CAC Login and be taken to the page where they can select MY PHA or DEPLOYER. If that doesn’t work, they need to select Create Account at the bottom of the Sign In screen.

  • Click “Deployer” under EDHA.

  • Select the “Report a New Assessment” button and follow the prompts in the pop-up window to associate your assessment with the correct deployment.

  • New Assessments can also be started by clicking the “Start” button under the available assessment.

  • Complete the survey by filling out all required fields.

*  If a page with required information is not completely filled out, an orange dot will appear next to that page. When a page is completely filled out, the orange dot will disappear.

  •  Finish the assessment and save. Assessments will not be marked as complete until they are certified.

  • Log out.

Step 4. Call to make an appointment

Pre/Post/PDHRAs must conducted in-person, after the online portion is complete; members can call the same numbers to schedule the in-person appoint for those. The Fleet and Family Medicine Clinic (F2) hours are 7:30 a.m. - 4 p.m. Monday through Friday.
