Medical Readiness Guidance

PHA:

• Go to  https://eha.health.mil/eha (CAC enabled log in)
• Select “My PHA” on left-hand side, proceed to answering all the questions.
• Once complete, contact HM2 Harris @ 757-953-6278 or HM3 Wentling @ 757-953-6281 to schedule a virtual PHA appointment with a provider.
• Per ALNAV 015/23, DON Service Members will complete their PHA during their birth month, unless operational requirements preclude its completion. Due annually.

HIV:

• Service member will be notified if an active HIV order is in their chart.
• Walk-in to a clinic with a lab to get blood drawn. 
• Clinic locations that accept walk-in are listed below.

Immunizations:

• Call Hampton Roads Appointment Center @ 1-866-645-4584 to schedule an appointment for delinquent immunization(s).  *Inform them of the specific immunization.*
• If unsure of delinquencies, contact HM2 Harris or HM3 Wentling.

Audiogram:

• Contact HM2 Harris or HM3 Wentling to schedule an audiogram appointment or determine when walk-ins are accepted.
• Tuesdays are scheduled audiogram days for NW Clinic, walk-ins are accepted based on staffing.

Dental:

• For Dental Services please report to the Boone Dental Clinic during normal working hours.
  Mon–Fri: 6:45 a.m. to 3:15 p.m.
  (sick call hours Mon–Fri: 7 a.m. to 12 p.m.)
