Preparing for Your Appointment
Contact the hospital’s free Child Waiting Center at (757) 953- 6904 and make a reservation for any child without an appointment. Confirm the clinic’s policies when you make your reservation.
Also bring copies of any completed medical or developmental/ educational evaluations.
If you already have an Individual Family Service Plan (IFSP), Individualized Education Program (IEP), or Individual Service Plan (ISP), please bring a copy with you.
For returning patients, a follow-up questionnaire may be used to tell us how your child has been doing since the last visit. Let us know if anything has changed with your child or with your own situation. Provide us copies of any medical or developmental/ educational evaluations completed since your child’s last visit.
On the Day of Your Appointment
Allow sufficient travel time to arrive at least 15 minutes before the appointment time. Please note that traffic in tunnels and parking on the installation may create significant delays. If you are more than 15 minutes late, you may be asked to reschedule your appointment.
If possible, have BOTH parents (or parent and a concerned other adult) attend the visit. It helps to have two people to listen and absorb new information.
If you need to cancel an appointment, if possible, please provide at least 48 hours advance notice.
Please DO NOT bring other young children (except nursing infants). If you do so, your appointment may have to be rescheduled. We want to devote the best care possible to your child. Other children detract from the quality of the evaluation.
