Ophthalmology Clinic
 

Ophthalmology is a specialty service that provides care for medical and surgical diseases of the eye. All new patients are required to have a referral from their Primary Care Manager (PCM) before an appointment can be made.
 
Once the referral has been submitted, patients should call the Hampton Roads Appointment Line at
1-866-645-4584 to check the status.  Please allow at least 48 hours (not including weekends or holidays) for review and disposition.
 
**Civilian PCM referrals to Ophthalmology are not processed through our department and cannot be viewed by the Hampton Roads Appointment Line** To inquire about referrals submitted by a civilian PCM, please call Tricare Humana directly at 1-800-444-5445 for status.  
 
If the referral is appointed for care at the Military Treatment Facility (MTF), the Appointment Line can schedule your appointment.  If the referral is deferred to the network, please contact Tricare Humana at 1-800-444-5445 for information on authorization for civilian care.
 
This clinic does not provide routine eye exams or exams for glasses / contact lenses.
