General Development Clinic (3 yrs or younger)
In order to make your child’s clinic visit as complete and as thorough as possible it is important that we have information about your child’s development and behavior. Please download and complete the following forms. Once you have completed them, send them along with copies of ANY previous evaluation (developmental, school, speech/language, occupational therapy, physical therapy), Individual Family Service Plan (IFSP). Our clinic scheduler will contact you to make an appointment once this information has been received.
