General Development Clinic (4 yrs and older)

In order to make your child’s clinic visit as complete and as thorough as possible it is important that we have information about your child’s development and behavior. Please download and complete the following forms. Once you have completed them, send them along with copies of ANY previous evaluations (developmental, school, speech/language, occupational therapy, physical therapy), Individual Education Plan (IEP) (if your child is receiving Special Education Services) and please e-mail, fax them or mail them. Our clinic scheduler will contact you to make an appointment once this information has been received.
